Our Services

Comprehensive Support, Tailored to Your Family

Straight Path CFS coordinates and provides supports for adults who reside in the family home — delivered with respect for each person's language, culture, and unique needs.

How CFS Is Delivered

CFS is tailored to the unique needs of the individual and their family and provided in a manner that respects their language and culture. Services are primarily provided in the person's home. CFS does not replace or duplicate any regional center service coordination, generic service, or other regional center funded service the individual and their family are already receiving.

At minimum, the need is assessed annually, with progress reported quarterly.

Important to Know

CFS shall not replace or duplicate any regional center service coordination, generic service, or other regional center funded service the individual and their family are already receiving.

Provided outside the home by an independent provider

CFS may not be provided by an individual who resides in the same home as the individual receiving services.

Annual assessment, quarterly progress reports

At minimum, the need for CFS is assessed annually, with progress being reported quarterly to ensure services remain aligned with the individual's goals.
