About Straight Path CFS

A New Kind of Support for Families Who Care

Straight Path Coordinated Family Services is built around one belief: adults with disabilities deserve to thrive at home, surrounded by the people who love them.

What Is Coordinated Family Support?

Coordinated Family Support (CFS) is a service option for adults ages 18 and older who choose to live in their family home and are served by a regional center. CFS helps adults coordinate the services and supports they need to remain in their family home — with dignity, flexibility, and respect.

Who Is Eligible for CFS?

CFS is for adults (18 years and older) who live with their family and are receiving services from a Regional Center.

  • Adults ages 18 and older
  • Currently living in their family home
  • Receiving services from a Regional Center

How CFS Helps

CFS providers create a personalized service plan to help reduce or remove barriers to staying in the family home, address challenges, and meet identified goals.

1

Developing Skills at Home

We work with individuals to build the skills they need to live more independently and confidently within their family home.

2

Coordinating Service Delivery

We coordinate and arrange the delivery of services — including assistance accessing generic services — so families don't have to navigate the system alone.

3

Community Connection

We help individuals connect to and participate in their community, building relationships and a sense of belonging beyond the home.

Our Approach

Services are delivered in a way that respects each person's and their family's language and culture. Every service plan is tailored, individualized, and flexible — designed to reflect the changing needs and preferences of the individual and their family over time. We don't believe in one-size-fits-all. We believe in your family's path.

Person-centered

Every plan starts with the individual's goals and preferences.

Culturally responsive

We honor each family's language, culture, and home environment.

Flexible & adaptive

Plans evolve as needs and circumstances change.

Community-focused

We support participation and belonging beyond the home.

Our Mission

Straight Path, in partnership with the Regional Center, is to provide people with developmental disabilities the opportunity to live safely and successfully either on their own or with their family/children in their own homes, within their own communities.

1

Self-Determination

We promote and encourage self-determination in a supportive, friendly environment.

2

Teaching Through Everyday Life

We help individuals learn by engaging them in the routines and responsibilities of daily living.

3

Modeling and Support

We act as positive role models for each person and look for every opportunity to teach appropriate behavior through modeling, prompting, and reinforcement — so each individual can lead a full and productive life.
