Contact & Referrals

Let's Find Your Path Forward

Whether you're a family exploring CFS for the first time or a regional center case manager making a referral, we're here to help.

How Can I Sign Up for CFS?

If you live at home with your family and think you need CFS, the process starts with your Regional Center Service Coordinator.

1

Talk to Your Service Coordinator

Let your Regional Center Service Coordinator know you're interested in CFS. They will schedule an IPP (Individual Program Plan) team meeting to determine how CFS can assist you and your family.

2

Receive a Referral

If you are referred for CFS, Straight Path CFS will meet with you and your family to talk about what you want to achieve by using CFS.

3

Build Your Plan Together

Together, Straight Path — along with you and your family — will prepare a personalized plan that identifies which services you need assistance with and how Straight Path CFS will help.

CFS is a regional center funded service. Your Service Coordinator can confirm your eligibility and initiate the referral process.
